Pacientes com doen\u00e7as neuromusculares heredit\u00e1rias (DNMs) ser\u00e3o avaliados atrav\u00e9s de triagens gen\u00e9ticas espec\u00edficas usando <em>Multiple Ligation Probe Amplification<\/em> (MLPA) para descartar condi\u00e7\u00f5es como Atrofia Muscular Espinhal (AME), distrofia muscular de Duchenne\/Becker e neuropatias Charcot-Marie-Tooth, ou sequenciamento de muta\u00e7\u00f5es recorrentes associadas a s\u00edndromes miast\u00eanicas cong\u00eanitas e miopatias mitocondriais. Espera-se que essa triagem inicial diagnostique aproximadamente 20% dos casos. Casos n\u00e3o diagnosticados por esse m\u00e9todo passar\u00e3o para o sequenciamento completo do exoma.<\/p>\n<p>Na primeira etapa, os exomas ser\u00e3o analisados com foco em um painel de mais de 600 genes relacionados a doen\u00e7as neuromusculares (baseado em <a href=\"http:\/\/www.musclegenetable.fr\/\">http:\/\/www.musclegenetable.fr\/<\/a>), que ser\u00e1 atualizado a cada 6 meses. Espera-se que essa abordagem resolva 40-60% dos casos. Para casos n\u00e3o resolvidos, ser\u00e1 realizada uma an\u00e1lise mais aprofundada do exoma completo, incluindo o sequenciamento de membros da fam\u00edlia, quando poss\u00edvel. Essa abordagem n\u00e3o s\u00f3 facilitar\u00e1 o diagn\u00f3stico de doen\u00e7as raras, mas tamb\u00e9m a identifica\u00e7\u00e3o de novos genes. Em tais casos, ser\u00e3o realizados estudos para validar a patogenicidade das variantes identificadas e confirmar sua relev\u00e2ncia utilizando diversos crit\u00e9rios, incluindo an\u00e1lises in silico e estudos funcionais em modelos celulares ou animais.<\/p>\n<p>No Latin-SEQ, a interpreta\u00e7\u00e3o e an\u00e1lise dos resultados gen\u00e9ticos seguem um processo meticuloso e rigoroso: os dados obtidos s\u00e3o analisados iterativamente e os resultados preliminares s\u00e3o compartilhados com os m\u00e9dicos respons\u00e1veis. Esses resultados s\u00e3o discutidos em reuni\u00f5es regulares para avaliar as descobertas com base na apresenta\u00e7\u00e3o cl\u00ednica do paciente e da fam\u00edlia. Para casos considerados resolvidos, ser\u00e1 emitido um relat\u00f3rio gen\u00e9tico final. Esses crit\u00e9rios incluem uma Frequ\u00eancia do Alelo Menor (MAF) &lt;1% na popula\u00e7\u00e3o controle, com base no gnomAD v4.0, e um impacto moderado ou alto segundo o Variant Effect Predictor (VEP). registros internacionais. Esses registros s\u00e3o respons\u00e1veis por compartilhar informa\u00e7\u00f5es com os pacientes e suas fam\u00edlias de acordo com seus pr\u00f3prios protocolos.<\/p>\n<p>A participa\u00e7\u00e3o em registros \u00e9 totalmente volunt\u00e1ria e fica a crit\u00e9rio do paciente e de sua fam\u00edlia. Registros de pacientes s\u00e3o ferramentas importantes que arquivam sistematicamente dados cl\u00ednicos e gen\u00e9ticos, com o objetivo principal de localizar pacientes para ensaios cl\u00ednicos. Os registros tamb\u00e9m servem como uma rede de apoio para os pacientes, oferecendo recursos adicionais que podem ajudar ao longo do curso da doen\u00e7a.
